Individualized case management is a personalized service that is available to families and individuals who want help reaching their housing, employment and/or educational desires in order to become self-reliant.   We believe in building on your family's strengths.

One of ESC's case managers will meet with you weekly to help you figure out how to met your goals. Appointments are scheduled in a location (or with COVID-19, virtual google hangout video meetings or by phone) where the individual/family feels comfortable. Goals are identified through partnership with the family.  Referrals are made to other community resources and programs that the individual or family requests.   Here is one successful customer.
